The solution is quite simple (i also experienced it), You need to install original driver for SD card reader that can be found under :

I don’t remember which one i used (for win10 or from original dump), but You simply need to manually “update” driver to one of those. And Your SD card stops disconnecting 🙂 You can find card reader in device manager window under USB devices tree.
